What is Endometriosis?

 

It is a condition where the endometrial tissue that is the lining of the uterus, is found outside the uterus. This happens due to retrograde flow of blood from the uterus upwards during menstruation.

Symptoms

Painful menses
 

Pain due to endometriosis starts one to two days before the periods and
continues during the periods.

 

Pain during sexual intercourse
 

Inability to conceive
 

Backache, pain related to bowel and bladder movements

Diagnosis

Clinical depending on the symptoms
 

Investigations include ultrasound . Presence of chocolate (endometriotic cysts) is a very common finding
 

MRI may be required for a more accurate diagnosis and staging of the disease .
 

Laparoscopy is the gold standard diagnostic modality . By putting a camera through key holes in the abdomen, the extent of endometriosis can be accurately known. .

Treatment
 

Pain relievers for controlling the pain -Hormonal treatment - Combined oral & contraceptive pills without the pill free break, Mirena that is an
intrauterine device containing a progesterone

 

Progesterone injection or pills

 

GnRH injections are very effective and cause a pseudo menopause like
state.


Surgical
 

Laparoscopic surgery can be offered to women with large endometriotic cysts
 

Even adhesions due to endometriosis can be handled laproscopically.
 

Patients trying to conceive usually require a surgical treatment since hormonal treatment cannot be offered to them .
